Referral-based virtual assessment of complex medical concerns, abnormal investigations and diagnostic or management questions that benefit from an integrated internal medicine perspective.
Reasons for referral
When the clinical picture crosses traditional boundaries
Internal medicine consultation can help organize complex information, refine a differential diagnosis and support a practical management plan.
Diagnostic questions
Unexplained symptoms or abnormal investigations requiring a broad adult-medicine assessment.
Medical complexity
Multiple chronic conditions, polypharmacy or competing treatment considerations.
Second-look review
Focused consultation where clarification, prioritization or additional recommendations may help.
Consultation approach
From information overload to a clear clinical plan
The consultation begins with the specific question from the referring clinician. Relevant history, medications, laboratory findings, imaging and previous specialist opinions are reviewed to identify priorities and reasonable next steps.
Recommendations may include additional investigation, medication review, risk management, monitoring, referral to another specialty or an in-person assessment when required.
This service does not replace longitudinal primary care or emergency assessment. Patients should continue routine and ongoing care with their primary-care clinician. Acute or unstable concerns require the appropriate local urgent or hospital pathway.
Referral-based specialist care across Ontario
Referring clinicians may submit relevant clinical information through the clinic’s referral pathway.
