Include current medications, recent laboratory results, imaging and relevant consultation reports.
Patient details
Provide current contact information, health-card information and any communication or accessibility needs.
Urgency
Clearly identify why a referral is urgent and include the clinical findings that support prioritization.
Urgent referrals are reviewed promptly and prioritized according to clinical need and appointment availability. This clinic does not provide emergency services or same-day emergency assessment.
After the referral is received
1. Clinical reviewScope, completeness, urgency and virtual-care suitability are assessed.
2. SchedulingThe clinic contacts the patient when the referral is accepted and ready to schedule.
3. Consultation reportFindings and recommendations are sent to the referring clinician.
